Offline Order Forms

Do any of you merchants offer an order form, or pdf downloadable form for your customers that insist on ordering "offline".?
We have developed a prototype and would like opinions!

We definitely have customers that insist on ordering offline, but we just use the "offline payment" option so they can fill their cart and enter their address as normal, but then simply choose that method and send in their money order. Seems to work pretty well.

I'm just wondering, however, how that works with inventory counts? Are the products still deducted, and if they are, what if the customer never sends in payment? Do you give them a time limit for payment? Just wondering, as we only keep a few of each item in stock at any time, and would hate to lose a sale to another customer due to an out of stock situation. Just wondering :)

If they choose offline payment it goes into "unpaid" and does indeed remove the item from inventory. We just have to keep our eye on them and cancel the order if we don't get payment. We don't have any set time to leave it in there... we just play it by ear.

We plan on printing order forms based on fun food type, and including in machine shipments. For example, purchase a popcorn popper and the popcorn supplies order form is included. And, make the forms available online.
However, a blank form with manual entry of part number will be needed if we were to include ALL our products, which number nearly 5,000. We haven't figured that one out yet. With so many variables on ship costs, weight, etc, a generic form is just not possible, yet......
