Responsive Broke Wholesale Page

One of our forums members did some work on my website awhile back which allowed retailers to order wholesale from my page. It worked great but just broke with a responsive template upgrade. I've been trying to reach the developer but in the meantime wondering if how hard of a fix this is going to be. 3dcart said the code wasn't HTML5 so will need some rework. Kind of need to get this fixed pretty quickly so could use the feedback.
